Purpose of this form
This form is intended for use by University of Sydney staff and students and external parties to report cases of modern slavery that may be linked to any part of the University’s operations or functions, including:
- suspected incidents of modern slavery, labour exploitation, debt bondage, or risks of forced marriage or other types of modern slavery, or if you are worried about someone who might be in this type of situation;
- concerns regarding modern slavery occurring, both on or off campus, or in the University’s supply chain through the procurement of goods and services.

If you feel unsafe on campus or are concerned for someone else’s safety on campus you can contact Protective Services on 9351 3333, 24 hours a day.
Privacy statement
We will collect your personal information when you voluntarily complete this form to report a modern slavery incident or concern related to the University of Sydney (the University). If you do not provide all the information requested, the University may not be able to adequately respond to your report.
We may also collect personal information (including sensitive information) about you indirectly from third parties, when we are complying with our obligations to assist the Anti-Slavery Commission, including the Commissioner and relevant government agencies.
The University may be required or authorised by law to disclose any personal information you provide with:
- The NSW Anti-Slavery Commissioner
- emergency or support services, or
- law enforcement agencies.
We will not otherwise disclose the personal information outside the University without your express consent.
While not required, if you do not provide your contact details the University may not be able to offer you support or provide any additional follow up action it deems appropriate.
The information you provide will be stored in the University's record keeping system and will be used to investigate and respond to your report. Identified information will only be accessible to staff whose duties relate to the University’s compliance with the Modern Slavery Act 2018 (NSW) and relevant systems support.
The information you provide may also be used by the University on a de-identified basis to detect trends and develop targeted responses to addressing issues related to modern slavery in the University.
You have the right to access and correct personal information about you held by the University. If you have any questions or concerns about this form, or if you wish to access or correct personal information held about you, please contact anti.slavery@sydney.edu.au
The University will handle your personal information in accordance with its Privacy Policy 2017 and Privacy Procedures.
